چکیده

A quantitative radionuclide angiogram technique using an intravenous injection of technetium99m (Tc-99m) pertechnetate is described for quantitation of left-to-right shunts in the central circulation. Data analysis assumes exponential indicator clearance from normal cardiac chambers by dilution. A late prolongation of tracer disappearance compared to the initial clearance rate indicates an abnormally early return of indicator to the cardiac chamber. Patients with left-to-right shunts demonstrate prolonged clearance of radioactivity from all cardiac chambers distal to the site of shunt. The magnitude of curve distortion is quantitatively related to the size of shunt, and counts recorded from the right lung are used for shunt quantitation. Quadratic regression analysis of data from 21 patients with left-to-right shunts and eight normal subjects demonstrated

Radionuclide quantitation of right-to-left intracardiac shunts in children.

A simple, noninvasive method for measurement of right-to-left intracardiac shunts would enhance the management of patients with congenital heart disease. This study describes application of data processing techniques used previously in dye-indicator curves to data recorded during the initial transit of radioactive bolus through the central circulation. Radionuclide angiocardiograms were perform...

Quantitative radionuclide angiocardiography: determination of Qp: Qs in children.

A new method of determining pulmonary-to-systemic flow ratios (Qp/Qs) in patients with left-to-right shunts using radionuclide angiocardiography is described. It involves the analysis of pulmonary time-activity histograms using a gamma-variate model. It appears to be simple, relatively atraumatic, and superior to the methods using C2/C1 ratios because, in addition to accurately detecting the pr...

Re: deconvolutional analysis in radionuclide quantitation of left-to-right cardiac shunts.

A poor bolus injection results in an unsatisfactory quantitative radionuclide angiocardiogram in as many as 20% of children with possible, left-to-right (L-R) cardiac shunts. Deconvolution analysis was applied to similar studies in experimental animals to determine whether dependence on the input bolus could be minimized. Repeated good-bolus, prolonged (greater than 2.5 sec), or multiple-peak i...

Superiority of radionuclide over oximetric measurement of left to right shunts.

In 100 children with suspected left to right shunts the ratio of pulmonary to systemic flow was measured both by oximetry and first pass radionuclide angiography. The pulmonary time activity curve from the radionuclide study was analysed by the method of gamma variate fits. There was strong correlation between the two techniques; weaker correlation was found when the shunt was at atrial rather ...

Noninvasive quantitation of right-ventricular volume overload in adults by gated equilibrium radionuclide angiography.

R-wave-synchronized equilibrium radionuclide angiography (RNA) is a noninvasive method whose time-activity curves provide count information that is proportional to ventricular volume. We have performed resting gated RNA in nine consecutive adult patients undergoing cardiac catheterization for evaluation of left-to-right shunting. Pulmonary/systemic flow ratios (Qp/Qs) calculated from RNA correl...
